Pan Card for below 5 years is called Child PAN Card. Child PAN Card do not have Applicant's Photo on it. It has parent's/guardian's signature on it. It has PAN number printed on top and contains PAN holder's name, date of birth, father's name. Most important, it also has Hologram which makes it genuine ID proof for its holder. For Child PAN Card following documents are required:

1. Aadhaar card

2. Birth certificate issued by the Municipal Authority or any office authorised to issue birth

4. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking

8. Bank account statement less than 3 months old.
9. Landline/Broadband connection bill less than 3 months old.
10. Electricity Bill less than 3 months old.
11. Election Card / Voter ID.
12. Post office passbook having address of the applicant.
13. Certificate of identity in Original signed by a Gazetted officer.
14. Driving License.
15. Aadhaar Card.
16. Certificate of identity in Original signed by a Member of Parliament.
17. Water Bill less than 3 months old.
18. Passport.
19. Depository account statement less than 3 months old.
20. Employer certificate in original.
21. Consumer gas connection card or book or piped gas bill.
22. Credit card statement less than 3 months old.

Any one of Child's Parent or Guardian needs to sign the PAN Card Form. If he/she is unable to do sign then left thumb impression is required on form.

Child PAN Card will be delivered to Parent or Guardian's Address by Courier mentioned in PAN Form while applying pan card for Child online. Address proof having same address needs to be submitted along with application.
